By the last Friday in August, faculty should submit via email a copy of their completed
MYC Summer/Fall Addendum Form and Board Report (sent via the app) to their Associate
Dean/Director for approval.
Deans will submit the completed application packet containing the following to Human
Resources by the second Friday in October:
(1) the signed MYC Candidate Form,
(2) the MYC Summer/Fall Addendum & Board Report Forms,
(3) at least two annual performance appraisals (including yearly goals and goal achievement
status), AND Faculty Teaching [Load] Schedules for the two year appraisal timeframe,
(4) student evaluations for the past four long semesters,
(5) class visit(s) completed by the associate dean/director.
Board Report Access is done in the app. Supervisors will upload electronic copies
to the MYC faculty folder.
NOTE: Deans should include two classroom teaching evaluations for applicants who have not
yet received two multi-year contracts; however, once a faculty member has earned two
multi-year contracts, then only one classroom teaching evaluation is necessary per
contract period thereafter. (This applies to both multi-year contracts and contract
extensions).
By the Fourth Friday in October, the packets will be ready for CoE review.

Note: A faculty member in his/her first multi-year contract is not eligible for a contract extension. All faculty members must go through the CoE process twice before becoming eligible for a multi-year contract extension. This process only applies to faculty members that have already been through the CoE process twice and have successfully received multi-year contracts after both reviews.
